The conference “From care to support and inclusion in the community: person-centered care”, promoted by the Department of Care and Social Policies, is a day of good practices that, from a knowledge management and learning approach, aims to disseminate, share and continue building a network of these fundamental principles in the transition of the care model.

This meeting, in addition to framing the model in its implications for professional practice, focuses on innovation, continuous improvement, evaluation and quality of care. It also aims to recognize and give visibility to some of the good practices in community centers and services of the territory that contribute to this change towards a more person-centered model of care.
