There is evidence that most of us want to grow old at home. However, as we get older and require more support, the limitations of our context make it difficult to continue our life project. To avoid this type of situation and to make it easier for people who need support to continue living in their environment and to live as they wish to live, it is necessary to rethink and reorganize the current care system and model.

The main objective of this project is to explore the needs of people in need of care in order to make continuous care pathways possible and to set up a local ecosystem of care, through the articulation of the public, private and community spheres aligned with the principles and guidelines of a co-created caregiver village agreement based on the rights of the people who care and are cared for.

In this first phase of the project, a diagnosis of the care system in Urnieta is being carried out, gathering needs and opportunities identified from the different spheres of the ecosystem (community, services, people in need of support, and caregiving families). Likewise, from the Social Services, some people are being attended through the case management methodology, that is to say, they will be monitored through a continuous care plan, focusing on knowing more closely the reality of these people and responding to their needs in a coordinated manner from the different spheres and resources for the provision of care. In addition, an action plan will be co-created in order to continue working along these lines in the coming years.

Funding:
Next Generation EU funds, within the framework of the Recovery, Transformation and Resilience Plan. Part of these funds have been allocated to social innovation projects in the Historical Territory of Guipuzkoa.
